What to expect next:

A staff member will contact you within 5-10 business days acknoledging the complaint. An investigator may also request a statement or additional information. The complaint form should include the following information:

The statement should include:

• Where the incident occurred
• Why you are making the complaint
• Witnesses (other colleagues, members of the public, other agencies) to the incident
